Platelet rich fibrin (PRF) is a second-generation autologous platelet concentrate derived from the patient's own blood. It consists of a three-dimensional cross-linked fibrin matrix that traps platelets, leukocytes (white blood cells), cytokines, and adhesive proteins such as fibrinogen, fibronectin, vitronectin, and thrombospondin-1[1][2][4]. PRF is prepared by drawing blood into tubes without anticoagulant and centrifuging it to separate its components; the resulting middle layer forms a gel-like clot rich in platelets and growth factors[2][5]. Unlike earlier products like platelet-rich plasma (PRP), PRF does not require additives or anticoagulants during preparation[5]. The mechanism of action involves the slow release of growth factors—including PDGF (platelet-derived growth factor), TGF-β (transforming growth factor beta), VEGF (vascular endothelial growth factor), EGF (epidermal growth factor)—and cytokines from the trapped platelets and leukocytes within the natural fibrin scaffold over an extended period. This sustained release supports tissue regeneration by mimicking physiological wound healing processes[1][2][6]. The presence of white blood cells also contributes to immune modulation at the site of application. Clinically, PRF is used as a biocompatible scaffold for tissue engineering in oral/maxillofacial surgery, dentistry for bone grafts or periodontal regeneration[4][10], orthopedics for cartilage repair or tendon injuries[1], dermatology/aesthetics for skin rejuvenation/hair loss treatments[5], and other regenerative medicine applications. Its autologous nature minimizes risks associated with immunological reactions or disease transmission.
