Overview

The combination of **platelet-rich fibrin (PRF)** and **botulinum toxin type A (BoNT-A)** is an investigational therapeutic approach primarily utilized in aesthetic medicine, regenerative dermatology, and scar management. BoNT-A is a neurotoxic protein produced by *Clostridium botulinum* that induces temporary muscle paralysis by cleaving SNAP-25, thereby inhibiting the release of acetylcholine at the neuromuscular junction. PRF is a second-generation autologous platelet concentrate prepared from the patient's own blood, forming a fibrin matrix that provides a sustained release of growth factors (such as PDGF, TGF-beta, and VEGF) and cytokines. The rationale for this combination is synergistic: BoNT-A reduces mechanical tension and dynamic muscle activity that can exacerbate wrinkles or scar formation, while PRF provides a biological scaffold and regenerative signals to enhance collagen synthesis, angiogenesis, and overall skin quality. Clinical studies have explored this combination for facial rejuvenation, the treatment of hypertrophic scars, and masseter hypertrophy.
