Platelet-rich plasma (PRP) is an autologous biological therapy derived from a patient's own blood, concentrated to contain a high density of platelets. Upon injection into the site of injury—in this case, the insertion of the Achilles tendon—platelets undergo degranulation, releasing a variety of growth factors including PDGF, TGF-β, and VEGF. These factors stimulate cellular processes such as chemotaxis, proliferation, and differentiation, which are essential for tendon repair and regeneration. Mahidol University is investigating the use of PRP as an adjunct to Extracorporeal Shockwave Therapy (ESWT) in a Phase 4 clinical trial to treat chronic insertional Achilles tendinopathy, aiming to improve functional outcomes and reduce pain compared to ESWT alone.
