Platelet-rich plasma (PRP) is an autologous biologic therapy prepared by concentrating platelets from a patient's own peripheral blood. In a clinical study led by the University of Michigan, PRP is administered via ultrasound-guided intratendinous injection for the treatment of gluteus medius and gluteus minimus tendinosis, which are primary causes of greater trochanteric pain syndrome. The therapeutic mechanism involves the release of high concentrations of growth factors—including platelet-derived growth factor (PDGF), transforming growth factor-beta (TGF-β), and vascular endothelial growth factor (VEGF)—from platelet alpha-granules upon activation. These factors act on their respective receptors to stimulate tenocyte proliferation, collagen synthesis, and angiogenesis, thereby promoting a regenerative healing response in the damaged tendon tissue.
