Platelet transfusion is a biologic intervention involving the administration of donor-derived platelet concentrates to patients with thrombocytopenia or platelet dysfunction. In a randomized controlled trial conducted by the Post Graduate Institute of Medical Education and Research (PGIMER), Chandigarh, this intervention was investigated as a therapeutic strategy to facilitate the closure of hemodynamically significant patent ductus arteriosus (PDA) in thrombocytopenic preterm neonates. The biological rationale is that platelets play a critical role in the anatomical closure of the ductus arteriosus by adhering to the endothelial wall, promoting thrombus formation, and releasing growth factors such as platelet-derived growth factor (PDGF) and transforming growth factor-beta (TGF-beta), which trigger vascular remodeling and permanent contraction. The PGIMER study specifically compared liberal transfusion strategies (maintaining platelet counts above 100,000 per microliter) versus restrictive strategies as an adjunct to standard pharmacotherapy with paracetamol or ibuprofen.
