plinabulin + pembrolizumab + etoposide + platinum is an investigational combination regimen being developed by BeyondSpring for the first-line treatment of extensive-stage small-cell lung cancer (ES-SCLC). The regimen combines plinabulin, a first-in-class small-molecule tubulin-disrupting agent that promotes dendritic cell (DC) maturation, with pembrolizumab, an anti-PD-1 monoclonal antibody checkpoint inhibitor, and standard-of-care chemotherapy consisting of etoposide and a platinum agent (carboplatin or cisplatin). The rationale behind this combination is that plinabulin-mediated DC maturation acts as a bridge between tumor neo-antigen generation from chemotherapy and T-cell activation enabled by PD-1 blockade, potentially improving the duration of response and overall survival. The regimen is currently being evaluated in an open-label, single-arm Phase 2 investigator-initiated trial (the '302 Study') in China.
