Overview

The PNET 5 MB chemotherapy regimen is a multi-agent treatment protocol used in the international prospective PNET 5 MB clinical trial for children and young adults with medulloblastoma. Developed and led by the Universitätsklinikum Hamburg-Eppendorf, the regimen is risk-adapted based on biological profiles, including WNT-activated, SHH-activated, and TP53-mutant subgroups. The protocol typically involves induction and maintenance phases using a combination of cytotoxic agents such as cisplatin, cyclophosphamide, vincristine, lomustine (CCNU), methotrexate (both high-dose and intraventricular), and doxorubicin (or carboplatin). The regimen aims to optimize survival outcomes while minimizing long-term toxicity by stratifying patients into low-risk, standard-risk, and high-risk groups based on molecular and clinical features.
