PNP-AV is a fusion protein designed for targeted enzyme prodrug therapy, specifically for metastatic prostate cancer. It consists of purine nucleoside phosphorylase fused with annexin V. The annexin V component targets phosphatidylserine, which is exposed on the surface of tumor vasculature and tumor cells. Once bound, the purine nucleoside phosphorylase enzyme locally converts a non-toxic prodrug into an active anticancer compound, thereby achieving high intratumoral drug concentrations while minimizing systemic side effects. Studies have shown its strong binding to prostate cancer cells and effective cell killing, with increased cytotoxicity when combined with low-dose docetaxel.
