Overview

Point-of-care washed red blood cell transfusion is a medical intervention being evaluated by the Mayo Clinic and Duke University to mitigate pulmonary complications associated with allogeneic blood transfusions in high-risk cardiac surgery patients. The procedure utilizes the Continuous AutoTransfusion System (CATS), an FDA-approved device, to wash allogeneic leukocyte-reduced red blood cells (RBCs) immediately before administration. This process aims to remove soluble biological response modifiers (BRMs), pro-inflammatory cytokines, and other components that accumulate during RBC storage, which are implicated in the pathogenesis of transfusion-related acute lung injury (TRALI) and transfusion-associated circulatory overload (TACO). By reducing the storage lesion components, the intervention seeks to improve clinical outcomes and reduce biomarkers of lung injury in patients predicted to require significant blood volume replacement.
