Polatuzumab vedotin plus rituximab, cyclophosphamide, doxorubicin, and prednisone (often abbreviated as Pola-R-CHP) is a combination chemotherapy regimen used primarily for the treatment of previously untreated diffuse large B-cell lymphoma (DLBCL), not otherwise specified (NOS), and high-grade B-cell lymphoma (HGBL). The regimen replaces vincristine in the traditional R-CHOP protocol with polatuzumab vedotin. Polatuzumab vedotin is an antibody-drug conjugate targeting CD79b on malignant B cells; it delivers monomethyl auristatin E (MMAE), a microtubule inhibitor, directly to these cells. Rituximab is a monoclonal antibody against CD20 on B lymphocytes. Cyclophosphamide and doxorubicin are cytotoxic chemotherapeutic agents with alkylating and anthracycline mechanisms respectively, while prednisone is a corticosteroid that exerts anti-inflammatory and immunosuppressive effects. This combination has been shown to improve outcomes in patients with moderate to high-risk DLBCL compared to standard regimens[1][3][5][7].
