Pv-R-CYM 1 + Pv-R-CYM 2 is a combination chemoimmunotherapy regimen consisting of polatuzumab vedotin (Pv), rituximab (R), cytarabine (CY), and methotrexate (M). It is being evaluated in the Phase 2 RADICAL trial (NCT05409222), sponsored by New York Medical College, for the treatment of children, adolescents, and young adults (CAYA) with newly diagnosed mature B-cell non-Hodgkin lymphoma (MB-NHL) or classical Hodgkin lymphoma (cHL). Polatuzumab vedotin is an antibody-drug conjugate (ADC) targeting CD79b on B cells, while rituximab is a monoclonal antibody targeting CD20. Cytarabine and methotrexate are small-molecule antimetabolites that inhibit DNA synthesis. The regimen is utilized as a consolidation treatment (Consolidation 2) in patients who show no residual disease after initial therapy, with the goal of assessing whether the addition of targeted immunotherapy can safely facilitate the reduction of toxic anthracycline exposure while preserving disease control.
