Drug intelligence / Profile preview
**Polatuzumab vedotin + rituximab + gemcitabine + oxaliplatin** is a four-drug combination regimen studied as a treatment for transplant-ineligible patients with relapsed or refractory diffuse large B-cell lymphoma (R/R DLBCL). The regimen, known as Pola-R-GemOx, includes: - **Polatuzumab vedotin**: an antibody-drug conjugate targeting CD79b, delivering the cytotoxic agent monomethyl auristatin E (MMAE) to B-cells. - **Rituximab**: a chimeric monoclonal antibody targeting CD20 on B lymphocytes. - **Gemcitabine**: a nucleoside analog (antimetabolite) that inhibits DNA synthesis. - **Oxaliplatin**: a platinum-based chemotherapeutic that forms DNA crosslinks. Recent phase III data (POLARGO trial) demonstrated that this combination provides a statistically significant improvement in overall survival versus the R-GemOx regimen, offering a potential new standard in heavily pretreated DLBCL patients. The main adverse events observed include cytopenias, infections, and neuropathy due to overlapping toxicities, particularly of polatuzumab vedotin and oxaliplatin[1][2][4][6].
