Polistes fuscatus venom protein is an allergenic extract derived from the venom of the northern paper wasp (*Polistes fuscatus*). It is indicated for the diagnosis and treatment of Hymenoptera venom hypersensitivity. As a diagnostic agent, it is used in skin testing (intradermal or prick) to identify IgE-mediated sensitivity in patients with a history of systemic reactions to wasp stings. As a therapeutic agent, it is used in venom immunotherapy (VIT) to reduce the risk of life-threatening systemic reactions (anaphylaxis) upon future stings. The extract contains major allergens such as phospholipase A1, hyaluronidase, and antigen 5. Immunotherapy works by inducing immunological tolerance, characterized by a shift from a Th2-mediated allergic response to a Th1/Treg-mediated response and the production of protective IgG "blocking" antibodies. It was approved by the FDA in 1992 (BLA 103971) and is marketed by companies such as Jubilant HollisterStier.
