Overview

Polyalkylimide is a synthetic, non-biodegradable, semi-permanent dermal filler primarily marketed under the brand name Bio-Alcamid. It consists of a cross-linked polyalkylimide hydrogel, typically composed of approximately 4% polymer and 96% apyrogenic water, derived from acrylic acid. Unlike traditional pharmaceutical drugs that interact with biological receptors, polyalkylimide functions as a medical device or soft-tissue endoprosthesis that provides volume through physical displacement. Upon subcutaneous injection, the material is encapsulated by a thin layer of host collagen (approximately 0.02 mm thick) over the course of about one month, which serves to stabilize the gel and prevent migration. It has been used extensively to treat HIV-associated facial lipoatrophy and for various cosmetic enhancements, including the correction of deep wrinkles, nasolabial folds, and depressed scars. However, its use has become controversial due to reports of late-onset complications such as chronic infections, granulomas, and migration, leading to its withdrawal or restricted use in several markets.
