Overview

The combination of polyethylene glycol (PEG), ascorbic acid (vitamin C), and senna is a low-volume bowel preparation regimen used to cleanse the colon prior to colonoscopy. It combines the osmotic laxative effects of PEG and high-dose ascorbic acid with the stimulant laxative properties of senna. PEG and unabsorbed ascorbic acid act as osmotic agents that draw and retain water in the intestinal lumen, softening stool and promoting watery diarrhea. Senna is metabolized by colonic bacteria into active rhein anthrone, which stimulates the enteric nervous system to enhance peristalsis and downregulates aquaporin-3 expression to inhibit water reabsorption. This combination allows for a reduced volume of bowel preparation solution (typically 1 liter) compared to standard 2-liter or 4-liter PEG regimens, improving patient tolerability and compliance while maintaining high bowel cleansing efficacy.
