Polygonum multiflorum is a perennial climbing plant native to East Asia and widely used in traditional Chinese medicine (TCM) under the names He Shou Wu or Fo-Ti. The root is the primary medicinal part and has been used for centuries as a tonic believed to promote longevity, healthy aging, hair growth/blackening, liver and kidney function enhancement, blood cleansing, and treatment of various ailments such as anemia and constipation[2][3][4][5][6][7]. Modern pharmacological studies have identified over 100 chemical constituents in Polygonum multiflorum; key bioactive compounds include stilbenes (notably 2,3,5,4′-tetrahydroxystilbene-2-O-β-glucoside), anthraquinones (such as emodin), and physcion[5][6]. These compounds are associated with antioxidant activity; animal/in vitro studies suggest potential anti-aging effects via pathways like DAF-16/SIR-2.1/SKN-1 signaling[10], neuroprotection (possibly relevant for Alzheimer's disease or Parkinson's disease), cardiovascular protection (anti-hyperlipidemia/atherosclerosis), immunomodulation and anticancer properties[3][5][10]. However clinical evidence supporting these uses is lacking. Polygonum multiflorum can be hepatotoxic—acute liver injury has been reported with both raw and processed forms; nephrotoxicity/neurotoxicity/embryotoxicity are also possible adverse effects[1][3][6]. Processing methods in TCM aim to reduce toxicity while enhancing therapeutic efficacy. The herb may interact with drugs metabolized by cytochrome P450 enzymes or antidiabetic agents due to its effect on hepatic metabolism/blood glucose levels[2].
