polymyxin B + bacitracin + lidocaine

A topical combination therapy consisting of the antibiotics polymyxin B and bacitracin (administered as Polyfax ointment) and the local anesthetic lidocaine (administered as Lignocain gel). This combination is used as a conventional treatment for second-degree burns to prevent infection and manage pain. Polymyxin B targets Gram-negative bacteria by disrupting their cell membranes, while bacitracin inhibits cell wall synthesis in Gram-positive bacteria. Lidocaine provides analgesia by blocking voltage-gated sodium channels, preventing the conduction of pain signals. This specific treatment regimen was used as the control arm in a clinical trial (NCT02497326) conducted by King Edward Medical University to evaluate the efficacy of topical heparin in burn management.
