Overview

Polymyxin b + doripenem is a combination of two intravenous antibiotics used primarily for the treatment of severe infections caused by multidrug-resistant Gram-negative bacteria, including carbapenem-resistant strains. Polymyxin B is a cationic polypeptide antibiotic that disrupts the bacterial cell membrane by binding to lipopolysaccharides and phospholipids in the outer membrane, leading to increased permeability and cell death. Doripenem is a carbapenem-class β-lactam antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), resulting in bactericidal activity. The combination has been studied for synergistic effects against pathogens such as Acinetobacter baumannii, Pseudomonas aeruginosa, Klebsiella pneumoniae, and Escherichia coli with various resistance mechanisms[1][2][3][4]. This regimen is considered when monotherapy options are limited due to resistance.
