**Polymyxin B sulfate + bacitracin zinc + hydrocortisone + neomycin sulfate** is a fixed-dose, sterile ophthalmic antibiotic-corticosteroid ointment marketed as **Cortisporin Ophthalmic Ointment**. Neomycin inhibits bacterial protein synthesis through the 30S ribosomal subunit; polymyxin B disrupts susceptible Gram-negative bacterial outer membranes through binding to lipopolysaccharide lipid A; bacitracin inhibits bacterial cell-wall synthesis by blocking recycling of the undecaprenyl pyrophosphate lipid carrier; and hydrocortisone activates glucocorticoid receptors to suppress ocular inflammation. It is indicated for steroid-responsive inflammatory ocular conditions in which bacterial infection is present or poses a meaningful risk, including inflammatory conditions involving the conjunctiva, cornea, and anterior segment, chronic anterior uveitis, and certain corneal injuries.
