**Polymyxin B sulfate + bacitracin zinc + hydrocortisone + neomycin sulfate** is a fixed-dose, sterile ophthalmic antibiotic-corticosteroid ointment marketed as **Cortisporin Ophthalmic Ointment**. Neomycin inhibits bacterial protein synthesis through the 30S ribosomal subunit; polymyxin B disrupts susceptible Gram-negative bacterial outer membranes through binding to lipopolysaccharide lipid A; bacitracin inhibits bacterial cell-wall synthesis by blocking recycling of the undecaprenyl pyrophosphate lipid carrier; and hydrocortisone activates glucocorticoid receptors to suppress ocular inflammation. It is indicated for steroid-responsive inflammatory ocular conditions in which bacterial infection is present or poses a meaningful risk, including inflammatory conditions involving the conjunctiva, cornea, and anterior segment, chronic anterior uveitis, and certain corneal injuries. The product has activity against several common ocular bacterial pathogens but does not provide adequate coverage against *Serratia marcescens*.
