PolyPEPI1018 is an off-the-shelf, multi-peptide cancer vaccine designed as an immunotherapy for metastatic colorectal cancer (mCRC). It consists of a combination of six synthetic polypeptides containing 12 immunogenic epitopes derived from seven tumor-specific antigens (cancer testis antigens) frequently expressed in colorectal cancers[1][3][7]. The vaccine is intended to stimulate the immune system by inducing broad T-cell responses against these tumor-associated antigens. Mechanistically, it acts as an immunostimulant that recruits and activates cytotoxic T cells to infiltrate tumors and convert "cold" tumors into "hot" ones—potentially enhancing anti-tumor immunity[7][10]. Developed by Treos Bio, it has been evaluated both as monotherapy and in combination with other agents such as trifluridine/tipiracil (TAS-102) or atezolizumab (a PD-L1 inhibitor)[3][5][6]. Clinical trials have shown that the vaccine is generally well tolerated with mostly mild local reactions; no serious or unexpected toxicities have been reported[5][10].
