Polythiazide is a thiazide diuretic used primarily for the management of hypertension and treatment of edema associated with conditions such as congestive heart failure, hepatic cirrhosis, corticosteroid and estrogen therapy, and various forms of renal dysfunction. It acts by inhibiting the solute carrier family 12 member 3 (Na+/Cl- cotransporter) in the distal convoluted tubule of the nephron in the kidney. This inhibition reduces reabsorption of sodium and chloride ions, leading to increased excretion of these ions along with water. The resulting diuresis helps decrease blood volume and lower blood pressure. Polythiazide may also increase potassium excretion via a sodium-potassium exchange mechanism. Its antihypertensive effect may be partially mediated through actions on carbonic anhydrases or large-conductance calcium-activated potassium channels in smooth muscle[1][2][6][8].
