Pomalidomide + bortezomib + dexamethasone is a three-drug combination regimen used primarily for the treatment of relapsed or refractory multiple myeloma, especially in patients who have previously received lenalidomide and a proteasome inhibitor. - **Pomalidomide** is an immunomodulatory agent that exerts anti-myeloma effects by activating proteasomal degradation of Ikaros and Aiolos transcription factors, modulating immune responses, inhibiting angiogenesis, and interacting with bone marrow stromal cells. - **Bortezomib** is a reversible proteasome inhibitor that disrupts protein degradation pathways in myeloma cells, leading to apoptosis. - **Dexamethasone** is a synthetic glucocorticoid corticosteroid with anti-inflammatory and cytotoxic properties that enhances the efficacy of the other agents. This triplet regimen (often abbreviated as PVd) has demonstrated improved progression-free survival compared to bortezomib plus dexamethasone alone in phase 3 clinical trials (e.g., OPTIMISMM trial). It is approved for use in adults with multiple myeloma who have received at least one prior therapy including lenalidomide[5][9][10].
