Pomotrelvir is a novel, orally active small molecule antiviral drug under clinical investigation for the treatment of COVID-19. It acts as a potent and selective inhibitor of the coronavirus main protease (Mpro or 3CL protease), an essential enzyme required for viral polyprotein processing and replication. Pomotrelvir demonstrates broad-spectrum activity against multiple human coronaviruses—including SARS-CoV-2 (including Omicron variants), MERS-CoV, and others—and shows high selectivity over human proteases. Clinical studies have shown that it is well tolerated with minimal drug-drug interaction potential due to weak inhibition of CYP3A4. Developed as an oral therapy for mild-to-moderate COVID-19 in non-hospitalized adults, pomotrelvir has completed Phase 1 and Phase 2 trials but did not meet its primary endpoint in otherwise healthy vaccinated adults without risk factors for severe disease[3][4][5][7].
