Overview

Ponesimod is a selective sphingosine 1-phosphate receptor 1 (S1P1) modulator indicated for the treatment of relapsing forms of multiple sclerosis (MS), including clinically isolated syndrome, relapsing-remitting disease, and active secondary progressive disease in adults. It works by binding with high affinity to S1P1 on lymphocytes, causing internalization and functional antagonism of the receptor. This prevents lymphocytes from exiting lymphoid tissues into the bloodstream, thereby reducing their presence in peripheral blood and limiting immune-mediated damage to nerves. Ponesimod is administered orally as a once-daily tablet[4][5][6][10]. The drug was developed to offer greater selectivity for S1P1 compared to earlier agents like fingolimod, potentially reducing off-target effects[4][5]. It was approved by the FDA in March 2021 for this indication[4].
