Ponesimod + carbamazepine refers to the co-administration of ponesimod, a selective sphingosine 1-phosphate receptor 1 (S1PR1) modulator approved for the treatment of relapsing forms of multiple sclerosis, with carbamazepine, a classic anticonvulsant and mood stabilizer commonly used for epilepsy, neuropathic pain, and bipolar disorder. Ponesimod acts primarily by functionally antagonizing S1PR1 on lymphocytes, resulting in reduced egress from lymph nodes and therefore reduced circulating lymphocytes, limiting their ability to enter the central nervous system and promote inflammation[2][3][4]. Carbamazepine is primarily a use-dependent blocker of neuronal voltage-gated sodium channels, stabilizing hyperexcited nerve membranes and suppressing repetitive neuronal firing. There are no fixed-dose combination products of these drugs; rather, this refers to concomitant oral administration, for which drug-drug interactions are clinically relevant. Carbamazepine is a strong inducer of CYP3A4 and UGT1A1, major enzymes involved in drug metabolism[3]. Co-administration is expected to decrease systemic exposure to ponesimod, potentially reducing its clinical efficacy, and such use is not recommended unless clearly necessary and with clinical monitoring[3].
