Overview

Posizolid is a synthetic oxazolidinone antibiotic developed by AstraZeneca and the National Institute of Allergy and Infectious Diseases. It acts as a small molecule inhibitor of bacterial protein synthesis by binding to the 23S ribosomal RNA of the 50S subunit, thereby preventing formation of a functional 70S initiation complex essential for translation. Posizolid demonstrated potent bactericidal activity against Gram-positive bacteria, including multidrug-resistant strains such as methicillin-resistant Staphylococcus aureus (MRSA) and vancomycin-resistant Enterococcus (VRE), as well as activity against Mycobacterium tuberculosis. Despite promising preclinical results and favorable pharmacokinetics, clinical development was discontinued after phase II trials due to negative trial outcomes and concerns about hematologic and hepatic side effects[1][2][5][6][8].
