Potassium iodide is an oral small molecule antithyroid agent primarily used to protect the thyroid gland from radioactive iodine exposure during nuclear radiation emergencies. It works by saturating the thyroid with stable (non-radioactive) iodine, thereby blocking uptake of radioactive iodine and reducing the risk of thyroid cancer and other thyroid damage. Potassium iodide is also used as an adjunct in treating hyperthyroidism (including thyrotoxic crisis), preoperatively to induce thyroid involution before surgery for thyrotoxicosis, and as an expectorant to loosen mucus in respiratory conditions such as asthma or chronic bronchitis. Its mechanism includes rapid inhibition of thyroid hormone release by decreasing thyroglobulin proteolysis and temporary inhibition of new hormone synthesis via the Wolff-Chaikoff effect[1][2][5][6][7].
