The pp65-flLAMP mRNA dendritic cell vaccine is an investigational autologous cell-based immunotherapy designed to treat glioblastoma and other high-grade gliomas. It consists of a patient's own dendritic cells (DCs) that have been pulsed with mRNA encoding the human cytomegalovirus (CMV) matrix protein pp65 fused to the full-length lysosomal-associated membrane protein (flLAMP). The flLAMP component is specifically designed to direct the pp65 antigen to the lysosomal compartment, thereby enhancing MHC class II presentation and stimulating a robust CD4+ T-cell response alongside CD8+ T-cell activation. Since CMV pp65 is highly expressed in glioblastoma cells but absent in normal brain tissue, the vaccine aims to induce a targeted cytotoxic T-lymphocyte response against the tumor. Developed primarily at Duke University, it is often administered with GM-CSF and site preconditioning with tetanus-diphtheria toxoid.
