PRaG 6.0 is an experimental combination immunotherapy regimen developed by the Second Affiliated Hospital of Soochow University for the treatment of advanced refractory non-small cell lung cancer (NSCLC) and breast cancer. The regimen is an evolution of the PRaG protocol (PD-1 inhibitor, Radiotherapy, and GM-CSF), expanded in version 6.0 to include metronomic vinorelbine chemotherapy and interleukin-2 (IL-2). This multi-modal strategy aims to enhance systemic anti-tumor immunity by combining local cytoreduction and antigen release (via radiotherapy and vinorelbine) with immune checkpoint blockade (PD-1/PD-L1 inhibition) and cytokine-mediated stimulation of dendritic cells and T-cells (via GM-CSF and IL-2). The approach is specifically investigated for its potential to induce abscopal effects in patients with refractory metastatic disease.
