Prajmaline is a semi-synthetic propyl derivative of ajmaline, classified as a class Ia antiarrhythmic agent. It acts primarily by blocking cardiac sodium channels in a frequency-dependent manner, thereby prolonging the action potential duration in cardiac tissue and suppressing arrhythmias. Prajmaline has higher bioavailability than ajmaline and is used to treat various cardiac disorders including coronary artery disease, angina pectoris, paroxysmal tachycardia, and Wolff–Parkinson–White syndrome. It is particularly indicated when other antiarrhythmic drugs are ineffective. The drug can be administered orally or intravenously and requires continuous treatment for sustained effect[1][4][5].
