Overview

Pralidoxime is a small molecule antidote in the oxime class, primarily used to treat poisoning by organophosphate pesticides and nerve agents, as well as acetylcholinesterase inhibitor toxicity. Its main mechanism of action is reactivation of cholinesterase enzymes (mainly outside the central nervous system) that have been inactivated by phosphorylation due to organophosphates. By cleaving the phosphate-enzyme bond, it restores normal neuromuscular transmission and relieves paralysis of respiratory muscles. Pralidoxime also slows the "aging" process of phosphorylated cholinesterase to a nonreactivatable form and can directly detoxify certain organophosphates. It is most effective when administered soon after exposure and is typically used alongside atropine for comprehensive management[1][2][3][4][5][6].
