Overview

Prasterone, also known as dehydroepiandrosterone (DHEA), is a synthetic small molecule steroid and an endogenous hormone precursor produced by the adrenal glands. It serves as an inactive metabolic intermediate that undergoes intracellular conversion into active androgens and estrogens, such as testosterone and estradiol, via tissue-specific steroidogenic enzymes. This mechanism, termed intracrinology, allows for localized hormonal action with minimal systemic impact. Marketed under the brand name Intrarosa as an intravaginal insert, prasterone is FDA-approved for the treatment of moderate to severe dyspareunia, a symptom of vulvar and vaginal atrophy associated with menopause. Beyond its approved use, DHEA is widely available as a dietary supplement and has been investigated for various conditions, including adrenal insufficiency, systemic lupus erythematosus, and potential lipid-lowering effects.
