Prasterone enantate is a synthetic steroid ester and a long-lasting prodrug of prasterone (dehydroepiandrosterone, DHEA), administered primarily by intramuscular injection. It acts as a weak androgen, estrogen, and neurosteroid. In the body, it is hydrolyzed to release active prasterone, which can be further metabolized into both androgens and estrogens. Prasterone exerts its effects mainly through agonism of androgen and estrogen receptors after conversion to more potent sex steroids such as testosterone or estradiol. It is indicated for hormone replacement therapy in postmenopausal women to treat menopausal symptoms such as moderate to severe dyspareunia due to vulvar and vaginal atrophy[1][2][4][5][6]. The drug has an extended duration of action (about 18 days per injection) due to its depot formulation. Prasterone also exhibits neurosteroid activity with potential effects on mood regulation and cognitive function[6]. Side effects may include symptoms of masculinization in women.
