Prednisolone + alfacalcidol is a combination therapy investigated by the **University of Aarhus** for the treatment of **minimal change disease** (MCN), a primary cause of nephrotic syndrome. Prednisolone is a synthetic glucocorticoid that functions as an anti-inflammatory and immunosuppressive agent by binding to the glucocorticoid receptor, thereby modulating gene expression to inhibit pro-inflammatory cytokines. Alfacalcidol is a synthetic analog of vitamin D3 (activated vitamin D) that acts as an agonist at the vitamin D receptor. This specific combination was evaluated in the ADAPTinMCN trial (NCT03210688) to determine if a reduced dose of prednisolone (0.5 mg/kg/day) combined with alfacalcidol could achieve non-inferior remission rates compared to standard high-dose prednisolone monotherapy (1 mg/kg/day) while minimizing corticosteroid-related adverse effects.
