Overview

This combination therapy consists of low-dose prednisolone, aspirin, and heparin, specifically investigated by Imperial College London for the treatment of pregnant women with antiphospholipid syndrome (APS) who have a history of recurrent miscarriage despite standard treatment. Prednisolone, a synthetic corticosteroid, acts as an immunosuppressant and anti-inflammatory agent by binding to the glucocorticoid receptor, potentially mitigating the autoimmune-mediated placental damage characteristic of APS. Aspirin provides antiplatelet effects through the irreversible inhibition of cyclooxygenase-1 (COX-1), while heparin (typically low-molecular-weight heparin) acts as an anticoagulant by activating antithrombin III, which subsequently inhibits thrombin and factor Xa. The combination aims to address both the thrombotic and inflammatory pathways involved in APS-related pregnancy loss, particularly in cases refractory to standard anticoagulant therapy.
