This therapeutic intervention involves the combination of systemic and local corticosteroid delivery for patients with chronic rhinosinusitis (CRS) undergoing endoscopic sinus surgery (ESS). Specifically, it consists of a postoperative course of oral prednisone (a systemic glucocorticoid) administered in conjunction with the placement of a triamcinolone-impregnated Nasopore spacer (a steroid-eluting middle meatal spacer). The regimen is designed to reduce postoperative inflammation, mucosal edema, and scarring. A clinical trial conducted by researchers at the University of Calgary, led by Dr. Luke Rudmik, investigated whether the addition of systemic prednisone provides incremental benefit over the local delivery of triamcinolone via the spacer alone. Both active ingredients, prednisone and triamcinolone, function as glucocorticoid receptor agonists to modulate inflammatory gene expression.
