PVAG is a combination chemotherapy regimen consisting of prednisone, vinblastine, doxorubicin (Adriamycin), and gemcitabine, specifically developed by the German Hodgkin Study Group (GHSG) and the University of Cologne for the treatment of elderly patients (aged 60-75) with advanced-stage or early unfavorable Hodgkin's lymphoma. The regimen was designed to mitigate the high toxicity associated with standard treatments like BEACOPP or ABVD in older populations while maintaining therapeutic efficacy. Gemcitabine, a nucleoside analog, serves as a key component by inhibiting ribonucleotide reductase and competing with dCTP for incorporation into DNA, leading to chain termination. Doxorubicin acts via DNA intercalation and topoisomerase II inhibition, vinblastine disrupts microtubule formation, and prednisone provides lymphocytolytic effects through glucocorticoid receptor activation.
