pregabalin + carbamazepine

This combination therapy consists of pregabalin and carbamazepine and is currently being evaluated in a clinical trial at the Postgraduate Institute of Dental Sciences, Rohtak, for the treatment of classical trigeminal neuralgia. Carbamazepine, the primary component, is a voltage-gated sodium channel blocker that stabilizes hyperexcited nerve membranes and inhibits repetitive neuronal firing. Pregabalin is added as an adjunct therapy; it is a GABA analog that binds to the alpha-2-delta (α2δ) subunits of voltage-gated calcium channels in the central nervous system, thereby reducing the calcium-dependent release of several excitatory neurotransmitters, including glutamate and substance P. The study aims to determine if the addition of a fixed dose of pregabalin (75mg twice daily) can enhance pain relief, improve quality of life, and reduce the required maintenance dose of carbamazepine, which is often limited by dose-dependent side effects such as drowsiness and hepatic dysfunction.
