Overview

Prophylaxis C is a combination of oral pregabalin and clonazepam being evaluated in the Phase 2 TALISMAN trial (NCT06500884) to prevent or minimize dysgeusia (taste changes) and other oral toxicities in patients with relapsed or refractory multiple myeloma receiving talquetamab. Talquetamab is a GPRC5D x CD3 bispecific antibody that frequently causes taste disturbances due to off-target effects on GPRC5D expressed in the oral mucosa. Pregabalin is an alpha-2-delta ligand that modulates voltage-gated calcium channels, while clonazepam is a benzodiazepine that acts as a positive allosteric modulator of the GABA-A receptor. This combination is administered orally starting before talquetamab step-up dosing and continues throughout the prophylaxis phase to mitigate neurological or sensory-related taste alterations.
