Premiplex (SHP607) is a recombinant human protein complex consisting of insulin-like growth factor 1 (rhIGF-1) and its primary binding protein, insulin-like growth factor-binding protein 3 (rhIGFBP-3). Originally developed by Premacure AB and later acquired by Shire (now part of Takeda), the therapy was designed as a systemic protein replacement to prevent retinopathy of prematurity (ROP) and other complications in extremely preterm infants. In utero, the fetus receives a continuous supply of IGF-1 from the mother; however, infants born very prematurely experience a sudden drop in IGF-1 levels, which is associated with abnormal vascular development in the retina and other organs. Premiplex aims to restore physiological IGF-1 levels to promote normal growth and prevent the pathological neovascularization characteristic of ROP. Despite reaching Phase II clinical trials, development was eventually discontinued by Takeda.
