Premylin-M

Premylin-M is a fixed-dose combination pharmaceutical product developed and marketed by Dios Lifesciences for the management of neuropathic pain. It combines pregabalin, an alpha-2 delta ligand, with methylcobalamin, a neurologically active form of vitamin B12. Pregabalin exerts its analgesic effect by binding to the alpha-2 delta subunit of voltage-gated calcium channels in the central nervous system, thereby reducing the influx of calcium and the subsequent release of excitatory neurotransmitters such as glutamate, norepinephrine, and substance P. Methylcobalamin acts as a cofactor for methionine synthase, facilitating the conversion of homocysteine to methionine, which is essential for the synthesis of myelin and the regeneration of damaged peripheral nerves. This dual mechanism aims to both alleviate pain symptoms and promote nerve repair in conditions such as diabetic neuropathy, postherpetic neuralgia, and spinal cord injury.
