Drug intelligence / Profile preview
Prenofitol is an investigational oral combination drug consisting of sodium polyprenylphosphate (a phosphorylated polyisoprenoid derived from coniferous needles) and beta-sitosterol (a plant-derived phytosterol). Developed by the Gamaleya Research Institute of Epidemiology and Microbiology in collaboration with the Ministry of Health of the Russian Federation, Prenofitol is designed for the treatment and prevention of dyslipidemia and metabolic syndrome. The drug exerts its lipid-lowering effects through a dual mechanism: beta-sitosterol competitively inhibits the Niemann-Pick C1-like 1 (NPC1L1) transporter in the small intestine to block cholesterol absorption, while sodium polyprenylphosphate stimulates interferon production, leading to downstream suppression of the SREBP2 transcription factor and inhibition of the mevalonate pathway. Additionally, sodium polyprenylphosphate directly inhibits 5-lipoxygenase (ALOX5) and 15-lipoxygenase (ALOX15), providing anti-inflammatory activity. Prenofitol is currently in Phase II clinical development in Russia.
