The application of pressure (typically 12 mmHg) facilitates deeper tissue penetration of the aerosolized drugs compared to traditional liquid intraperitoneal chemotherapy. In the specific protocol evaluated by The University of Hong Kong, PIPAC utilizes oxaliplatin for colorectal cancer-derived peritoneal metastases, or a combination of cisplatin and doxorubicin for gastric and pancreatic cancer-derived metastases. This approach is often used in a bidirectional manner, combining local PIPAC treatment with systemic chemotherapy to maximize therapeutic efficacy and potentially convert unresectable disease to a state suitable for curative cytoreductive surgery.
