Overview

Pretomanid is a small molecule antibiotic in the nitroimidazooxazine class, used as part of a combination regimen with bedaquiline and linezolid for the treatment of pulmonary extensively drug-resistant (XDR) tuberculosis or treatment-intolerant/nonresponsive multidrug-resistant (MDR) tuberculosis in adults. It acts as a prodrug that is metabolically activated by the mycobacterial enzyme deazaflavin-dependent nitroreductase (Ddn), leading to production of nitric oxide and other reactive metabolites. These metabolites inhibit mycolic acid biosynthesis under aerobic conditions, disrupting cell wall formation, and act as respiratory poisons under anaerobic conditions by releasing nitric oxide, which interferes with electron transport and ATP synthesis. Pretomanid was developed by TB Alliance, marking it as one of the first TB drugs developed by a nonprofit organization[1][4][6][7][8].
