Pretomanid + bedaquiline + quabodepistat (OPC-167832) + sutezolid is an investigational four-drug oral combination regimen being developed for the treatment of drug-sensitive and drug-resistant tuberculosis, including rifampicin-resistant and multi-drug resistant TB. This combination, also referred to as the PBQS or PAN-TB PBQS regimen, is currently in Phase 2 clinical trials as part of the PAN-TB Collaboration. Each component has a distinct mechanism of action targeting *Mycobacterium tuberculosis*: * Pretomanid is a nitroimidazooxazine that inhibits mycolic acid biosynthesis and releases reactive nitrogen species under anaerobic conditions, leading to bacterial cell death. * Bedaquiline inhibits mycobacterial ATP synthase, disrupting energy production in *M. tuberculosis*. * Quabodepistat (OPC‑167832) is a carbostyril derivative that inhibits decaprenylphosphoryl‑β‑D‑ribose 2'-oxidase (DprE1), an essential enzyme for cell wall biosynthesis. * Sutezolid is an oxazolidinone antibiotic that binds to 23S rRNA and inhibits protein synthesis by blocking formation of the initiation complex at the ribosome. The goal of this all-new oral regimen is to provide shorter, safer, and more effective treatment options for both drug-sensitive and drug-resistant forms of TB[1][3][8].
