Overview

Prexasertib + gemcitabine is an investigational combination of two drugs: prexasertib, a checkpoint kinase 1 (CHK1) inhibitor, and gemcitabine, a nucleoside analog DNA-damaging chemotherapeutic. Prexasertib inhibits CHK1, a key regulator of the DNA damage response, leading to increased DNA damage and apoptosis in tumor cells, especially when combined with DNA-damaging agents like gemcitabine. Gemcitabine works by incorporating into DNA and inhibiting DNA synthesis, leading to cell death. The combination has shown strong synergy in preclinical models and has been explored in phase 1 clinical trials for pediatric recurrent or refractory medulloblastoma, as well as in preclinical and clinical studies for acute lymphoblastic leukemia and pancreatic cancer. The combination aims to increase cancer cell death by simultaneously promoting DNA damage (via gemcitabine) and blocking the cell's ability to repair this damage (via prexasertib)[1][3][4][5][6].
