Overview

Primary BCMA-CAR T cells are a class of chimeric antigen receptor (CAR) T-cell therapies that target the B-cell maturation antigen (BCMA), a protein highly expressed on the surface of malignant plasma cells in multiple myeloma. Unlike induced pluripotent stem cell (iPSC)-derived or 'off-the-shelf' allogeneic products, 'primary' CAR-T cells are typically manufactured using T cells harvested directly from a patient (autologous) or a healthy donor. The T cells are genetically modified to express a CAR that recognizes BCMA, expanded ex vivo, and then re-infused into the patient to induce targeted anti-tumor cytotoxicity. This modality has revolutionized the treatment of relapsed or refractory multiple myeloma, with several products such as idecabtagene vicleucel and ciltacabtagene autoleucel having received regulatory approval.
