Prismocitrate 18 is a sterile, ready-to-use solution containing trisodium citrate at a concentration of 18 mmol/L (0.5%) and sodium at physiologic levels (140 mmol/L), designed specifically for use as an anticoagulant in continuous renal replacement therapy (CRRT) using regional citrate anticoagulation (RCA). It acts by chelating calcium in the extracorporeal circuit, thereby preventing blood clotting within the dialysis filter and circuit. After metabolism—primarily in the liver and skeletal muscle—the citrate is converted to bicarbonate, contributing to buffer balance. The low-concentration formulation reduces risks associated with hypertonic solutions such as metabolic complications or hypernatremia and allows higher infusion rates for efficient therapy delivery. Prismocitrate 18 is used as both a replacement fluid and an anticoagulant during CRRT modalities like CVVHDF or CVVH[1][2][3][6][7].
