Progesterone-induced blocking factor (PIBF) is a protein mediator produced by lymphocytes and trophoblasts in response to progesterone, playing a vital role in maintaining pregnancy by promoting maternal-fetal immunotolerance. It functions by shifting the immune response toward a Th2-dominant profile, suppressing natural killer (NK) cell activation, and reducing the production of pro-inflammatory cytokines such as TNF-alpha. In preclinical models of preeclampsia, such as the Dahl salt-sensitive rat, exogenous administration of PIBF has been shown to significantly reduce mean arterial blood pressure, decrease systemic inflammation, and improve fetal and placental weights. The University of Mississippi Medical Center is a primary institution investigating PIBF as a potential therapeutic intervention for preeclampsia and pregnancy-related hypertensive disorders.
