Proinsulin C19-A3 peptide is an 18-amino acid synthetic peptide derived from the human proinsulin molecule, specifically designed as an immunodominant epitope restricted by HLA-DR4 (DRB1*0401). It is being developed as a form of antigen-specific immunotherapy for type 1 diabetes. The mechanism of action involves modulating the immune response to reduce autoimmune destruction of pancreatic beta cells by promoting regulatory T cell responses and reducing pathogenic T cell activity. Clinical studies have shown that intradermal administration in newly diagnosed type 1 diabetes patients is well tolerated, does not accelerate beta cell loss, and may help stabilize insulin production and reduce daily insulin requirements. The therapy aims to induce immune tolerance specifically within pancreatic islets without causing systemic immunosuppression[2][5][6].
